Please fill the form out in its entirety. Whenever you approach areas that requires names and birthdates, it's important that you share what's on the passport. If you don't have a passport, please share what it "might" be. The inaccuracy of this information will cause issues in the long run.

Only 1 form is needed, per reservation. Whomever name the booking will be under, that's who should fill out the form.
